Haha, he is don't-be-evil-Google. And the "enviable honorific" facial expression is HILARIOUS. And the other facial expressions are HILARIOUS. And, of course one of the big big changes between where I used to live and where I live now is the private vs. non health-care, and I'm not up on all the related issues so I'm not hugely opinionated about it, but so many people have such very strong opinions and love to talk about them, and it really is a big difference and I think about it sometimes, so I can't tell if this strip is supposed to be some sort of commentary or I am just being influenced by the living-where-I-did/do-and-the-people-talking towards reading too much into this.
