I know with this game, more so than the others, each stage is determined by what you accomplish when. so, any time you acquire something new, or a different result happens, it can change things in many different areas, not just the one you are at, at the time.
with that said, have you already done all you can in the recovery room ? IF you've already used the washcloth on the old man, go to the lab, pharmacy area & see if someone is interested in that nickel. with what you get there, you'll be able to help the old man more.....so, he'll help you.
if you haven't helped him any yet, visit the recovery room, the washcloth will come in handy !
