Not to mention determining a setting... well, I'm a rabid worldbuilder, and if there's one thing I've learned, it's how even the smallest things in a setting can profoundly influence the storyline.

Not that I think we should go build a full-blown world first (unless you really want to; if so, I have four worlds to my credit and would be only too happy to help make another), but it would be nice to clearly say what the setting is. I've had really odd experiences with group stories where we thought everyone understood the fantasy/medieval tech level setting until one of the authors brought in Sailor Moon characters for no good reason.

But you probably already knew that. ;)
